Self-management programs are designed to allow patients to monitor and maintain their health, with minimal professional intervention. Numerous studies have proven the effectiveness of well-designed programs, showing them capable of dramatically improving patient’s health, and saving health care dollars.
Patients must be made to feel empowered. They must truly believe that they have the ability to make positive changes in their lives and pave the road to their own recovery. Many studies have been done that show the net benefits that result from multidisciplinary teams that work as a synergistic group, intervening at key moments in the recovery process, emphasizing and guiding the self-management process.
